This six month Care Coordination Academy aims to propel new care coordinators in their careers by imparting the crucial skills needed to directly and effectively address the needs of their patients.

This activity is intended to meet the needs of nurses and social workers.

  • Strategize communication within care coordination teams, with patients, families, and multi-disciplinary colleagues.
