Preventing Medical Errors

This activity addresses the identification, reporting, monitoring and reduction of medical error. It is intended to make participants aware of the magnitude of the problem, of the many-leveled activities needed to resolve it, and the importance of each person’s role in the national effort to reduce medical errors.

Needs Statement
The 1999 Institute of Medicine report To Err is Human: Building a Safer Health System brought prominence to the issue of medical error in the US. Since that time multiple activities at various levels of private and regulatory arenas have targeted identification and resolution of medical error. In 2016, researchers from Johns Hopkins University published findings from their study indicating that medical error can now be ranked as the third leading cause of death in the U.S. In 2020 most states are still mandating continuing education for licensed health care professionals. Health professionals and health systems both need ongoing education in error reduction techniques in order to create and maintain an improved culture of safety.

Target Audience

This activity is intended for physicians, physician assistants, pharmacists, nurses, nurse practitioners, psychologists, dentists, athletic trainers and other healthcare professionals at all levels and settings in the healthcare industry.

Learning Objectives

  • Recognize the magnitude and far-reaching effects of medical error.
  • Categorize factors contributing to the occurrence of medical error.
  • Describe the healthcare professional’s role in identifying error prone situations.
  • Explain the importance of reporting medical error.
  • Specify at least two recommended practice modifications to promote safety and reduce the incidence of medical error.
  • Review the processes of root cause analysis and failure mode and effects analysis when analyzing error.
  • Identify and review the five most misdiagnosed conditions.
