ForwardTSAOG: Trends and Strategies in Pain Management Coding
Interventional pain management is governed by restrictive payor coverage policies, subject to persistent audit scrutiny, and shaped by some of the densest bundling logic in the CPT® code set. In this session participants will examine how Medicare LCDs/LCAs and commercial medical policies define coverage criteria, frequency limits, conservative-therapy prerequisites, and documentation requirements for common pain procedures — and how those requirements differ.
Target Audience
Physicians, Non-Physician Providers, Billing Managers, Coding/Billing Staff, Administrators, and Revenue Cycle Directors.
Learning Objectives
Upon completion of the educational activity, participants should be able to:
- Identify the coverage criteria, frequency limitations, and documentation elements required by Medicare LCDs/LCAs and representative commercial policies for common interventional pain procedures
- Analyze documentation from interventional pain encounters to detect the deficiencies most frequently cited in audit findings and denials
- Assign CPT and HCPCS codes to multi-procedure pain encounters, sequencing the claim correctly and appending modifiers 59, X{EPSU}, 50, 51, and 76 as supported by documentation and payor policy
- Evaluate whether a proposed edit override is defensible, and justify the decision using the NCCI Policy Manual, payor policy, and operative documentation
