Spasticity and Movement Disorders IMPACT Initiative (IMproving Patient Assessment and Chemodenervation Treatment

The IMPACT workshop will provide an interactive and comprehensive opportunity to improve your knowledge, confidence, and proficiency in essential aspects of spasticity and movement disorder management. This workshop aims to enhance patient outcomes from chemodenervation procedures by improving participants’ ability to recognize clinical patterns, identify muscle targets, and perform procedures confidently and skillfully, including using ultrasound guidance in these procedures. 

For doctors, The Kinesthetic Training Sessions will cover ultrasound scanning and guidance. This will include hands-on ultrasound muscle identification exercises, injection practice with ultrasound phantoms, injection training with anatomical models (ELVIS, ELVIRA, ELISE; DAISEY, SADIE), and toxin “draw up” training. For physical therapists and occupational therapists, a dedicated track will focus on gross and clinical anatomy, with related ultrasound demonstrations, discussion of the principles of chemodenervation, a video example of splinting and casting, and methods for individualizing patient goals and management. All participants will have the opportunity to explore strategies for multidisciplinary care and will participate in the expert-led patient injection demonstrations of ultrasound-guided chemodenervation procedures. 

 

Target Audience

The activity is intended primarily for neurologists, physiatrists, and orthopedic surgeons (residents/fellows /community/academic physicians); nurses, nurse practitioners and physician assistants (working in neurology or PMR practices); physical and occupational therapists (specializing in neurorehabilitation)

 

Learning Objectives

MD Track

  • Describe the clinical presentation, diagnosis, and treatment of patients with cervical dystonia, limb dystonias, and spasticity 
  • Review the mechanism of action, indications, and clinical application of botulinum neurotoxin (BoNT) herapy in patients with cervical dystonia, limb dystonias, and spasticity  
  • Identify the indications and unique dosing requirements of the current BoNTs for the treatment of cervical dystonia, limb dystonias, and spasticity 
  • Demonstrate guidance techniques for the safe and effective injection of BoNT in patients with cervical dystonia, limb dystonias, and spasticity 
  • Identify sonoacoustic anatomical features of cervical and limb muscles, nerves, and other structures using ultrasound guidance to improve the safety and accuracy of BoNT injections
  • Develop strategies for individualizing therapy and assisting patients with cervical dystonia, limb dystonias, and spasticity in achieving their functional goals 

PT/OT Track

  • Recognize common patterns of muscle overactivity (MoA) that occur in patients with upper motor neuron syndrome and dystonia
  • Identify when patients with MoA should be referred for treatment (problematic MoA)
  • Recognize the importance of patient evaluation, collaborative care, and setting patient-specific goals on treatment outcomes
  • Demonstrate appropriate manual therapy, splinting, and casting techniques to enhance patient selection for and maximize functional outcomes in the post chemodenervation setting
  • Integrate strategies to individualize functional goals considering varying types of patient scenarios
  • Review the potential utility of ultrasound in management of upper and lower limb dystonias
